The FPA says you are on J5.0.2 (i.e. current) but you have J3 templates left on your site (Beez 3, Protostar, Isis, Hathor). These should have been removed during the J3.10.12 --> J4.x update process:
I'm also worried about the "?" after Cassiopeia and Atum templates.
Either you're using the wrong method to update or the update didn't complete properly. What are the last few lines of your update log (/administrator/logs)?
Also: "live_site" should be empty.
